The Wisconsin Coalition for Social Connection offers resources for individuals and professionals around social isolation and loneliness. They also have network meetings you can join! Check out the "about" page to see what organizations and individuals are in the coalition. 
I have spoken with the coordinator for WCSC and she has taken an interest in social prescribing (going as far as hosting a webinar about it in May 2025 -- See recording here)!
